So, as per assessment, um, patient's perineum seems to be a little red, there's a little bit of swelling, um, but there's no presence of foul smell. So the discharge is a bit pinkish to brownish and the amount is scanty. So by the end of this procedure, we will be able to identify if there are any signs and symptoms of infection, and we will ensure that the patient is comfortable. So good morning, Miss Smith. So for today, we will do perineal care. Um, this procedure will help us ensure that we clean any vaginal discharge, as this vaginal discharge might cause infection. So this procedure will also help prevent any infection. So for this procedure, Miss Smith, I will be exposing your perennial area and we will be applying um, waterproof pads underneath or um, just above the bed to ensure that you and your bed will not be soaked. Um, once the perineal area is exposed, we will clean it using an irrigating solution. And then um, we're just going to clean up everything and then put everything back to normal.
[1:42]Assemble all equipments. So we will just assume that this is the irrigating solution. And assume that this is the forceps. The cherry balls, the waterproof pad, the bed pad or bed pan, and the sterile perineal pad. It's okay, Miss Smith, I'm just going to um, expose your area, and I want you to flex your knee for me. Okay. And I'm just going to turn you to the side, so can you do it in your own or do you want me to assist you? Okay, so I will assist you, and I will put on the waterproof pad. So my patient is now in a dorsal recumbent position with the knees flexed and separated. I've exposed the perineal area, and I'm going to first um, separate the labia. And I'm going to pour a um, the irrigating solution. And with my um, forceps and cotton balls, we're going to first wipe the labia minora, which is inside. We're going to do the rule of sevens from the clitoris and down. And we're going to throw away this cotton. Then get a new one, and the other side now, and down. Okay. So after that, we're going to now wipe the labia majora. We threw away the old cotton and now we got a new one. And then wipe it again in the same manner, the rule of sevens, and throw away this one. Get a new one. And then the other side.
[4:36]Okay. We're also going to clean the middle part over here. Just downward and discard this cotton ball.
[4:48]So after cleaning with the irrigating solution, we're now going to dry it. Um, we've got new cotton balls again, and we just going to separate the labia again. And then we're going to first wipe the inner side dry first. So the labia minora first in one side and discard. Then get a new cotton ball and do on the other side the same way.
[5:19]And discard. Get a new one. Now for the labia majora.
[5:28]Discard. Get a new one and the other side. Okay. And a new one for the middle part. Okay. Now we're going to apply the sterile pad on the patient. And then we're going to wear her underwear. So after placing the client's underwear back to the same place, we're now going to cover her up with her blanket. Make sure that it's not wet, and then we're going to remove the pads underneath. So you can now sit in a comfortable position, but first let me remove the pads. Could you turn to your side again? Thank you.

[6:33]So after assessing the patient and doing the procedure, um, the discharge was pinkish to brownish and the amount was scanty, which is normal for her condition. And so we just document these findings and then put it into the chart. Now we will evaluate the procedure. So Miss Smith, did you feel any pain or uncomfortableness during the procedure? Okay, that's great. So as per the patient, she did not feel any pain or uncomfortableness, she was comfortable while doing the procedure. Um, there were no signs and symptoms of infections.
